PHD Network

PHD Network (Personal Health Risk Assessment and Self-Care Resources): The Personal Health Risk Assessment and Results are part of the Personal Health Development (PHD) Health Management Program provided to us by Duke Medical Center.

Access is only available to authorized participants and administrators and the privacy of each account is protected via unique Username and Password established by each User. The data collected by this assessment is used to describe health risks at an individual and group level, assign relevant health and wellness lessons to Users, and provide data for research by the Duke team.
The Personal Health Assessment (PHA) has approximately 130 questions that collect information on the User's current health, personal health history, family health history, lifestyle, and health habits. A User is required to answer the majority of the questions on the PHA prior to being able to successfully submit the form. Non-required questions include questions such as those that ask for specific laboratory values. All answers to PHA questions are referred to as Risk Factors throughout the PHD Network system.

Since the assessment is used to define a User's health risks and to personalize the education each User receives, Users are encouraged to answer each question as accurately as possible. The more information provided the more accurate and appropriate the current health and disease risk charts and lesson assignments will be. All assessment results are saved and Users are able to view these at any time through their on-line individual PHD Menu Page.

After completing the Personal Health Assessment, each User is presented with a Personal Health Chart (Red Box Chart) based on their responses to the questionnaire. This chart allows Users to quickly identify which modifiable Risk Factors are putting them at risk for developing or worsening a Disease/Condition. Relevant health and wellness lessons assigned to the User pop up as the cursor is moved over each red box.

Our goal is to provide important health and wellness information about individual health needs to improve quality of life. Each participant is assigned a program tailored to meet individual needs based on specific risk factors for a variety of health concerns. Members then interact with their unique series of programs to learn more about their specific health concerns and how to better manage their lives.  This comprehensive Health Management System helps guide both individuals and groups down the road to better health and wellness!
